This is a non-refundable, annual dental plan. The full membership fee is due upon enrollment, and coverage begins on the payment date. No refunds or credits will be issued if you choose not to use the plan. You will not receive a physical membership card; your effective date will be recorded in our system. Membership renewal is due annually, in the same month as your initial enrollment. Membership rates are subject to annual review and change with prior notice.
It is your responsibility to schedule and attend appointments within your 12-month membership period to fully utilize your benefits. Unused appointments will not be refunded. Routine appointments are scheduled at enrollment and subject to standard cancellation policies.
This plan cannot be combined with other offers or insurance. It does not cover: Workers' comp, specialist referrals, hospital charges, treatment outside the dentist's capabilities, or auto medical insurance claims. Valid only at American River Dental.
